In recent years, I’ve seen the popularity of dermal fillers in Nanaimo surge, and new products and technology are more innovative than ever before, offering immediate results and minimal downtime. But in the quest for smoother skin and enhanced facial contours, I receive many questions regarding the safety and longevity of these injectable treatments. As more people increasingly seek long-term solutions to combat signs of aging, new information has been published about the importance of monitoring dermal filler progression and, in some cases, dissolving filler that’s overstaying its welcome. In this article, I’ll provide insights into the latest research you need to know about dermal fillers, and give you the real facts on how long dermal fillers really last. 

What are dermal fillers? 

Dermal fillers are injectable substances used to add volume, smooth wrinkles, and enhance facial contours. I commonly use them in cosmetic procedures to address signs of aging, such as wrinkles, fine lines, and loss of facial volume. Dermal fillers can also be used to augment lips, cheeks, and jawlines, as well as to fill in acne scars or other depressions in the skin. The most common type of dermal fillers are made of hyaluronic acid, a substance naturally found in the body that helps maintain skin hydration and volume. Are dermal fillers safe?

For dermal filler procedures, I use Health Canada approved products and proper injection techniques, which are considered safe for cosmetic enhancement. While most clients experience only minor and temporary side effects such as bruising or swelling at the injection site, it’s important to note that more severe complications such as infection or allergic reactions are rare but possible. Because of this, I’m meticulous about pairing the right product with the needs of my clients, and staying vigilant about injection methods. Every treatment with me begins with a built-in consultation to discuss medical history, expectations, and potential risks before we proceed—I will never pressure you into proceeding with a treatment that you’re not 100 percent comfortable with. How long does it take for dermal fillers to work?

Dermal fillers typically provide immediate results, with noticeable improvements in volume, smoothness, and facial contours visible immediately following the injection. However, the full effect of dermal fillers may not be apparent until any associated swelling or redness subsides, which usually occurs within a few days to a week after treatment. You may experience some initial swelling or bruising at the injection site, but once these temporary side effects resolve, the final outcome of the treatment becomes more apparent. For many of my patients (particularly lip filler patients), the initial swell is quite shocking, but immediately missed once it subsides. Please wait a minimum of two weeks before booking in a follow-up or touch-up treatment as we need to allow the product to fully settle. 

How long do dermal fillers last? 

The duration of dermal fillers varies depending on several factors, including the type of filler used, the area treated, and individualized differences in metabolism. Generally, hyaluronic acid-based fillers, one of the most commonly used types, can last anywhere from six months to two years. These estimates are not definitive, and your response to treatment can vary. Factors such as your age, the treatment area, and lifestyle habits can also influence the longevity of filler results. I typically recommend regular maintenance treatments to maintain your desired volume and smoothness over time, as the body gradually absorbs the filler material. 

The Revanesse® line of dermal fillers is optimized for durability, lasting on average between 6-12 months but some areas wear faster than others. For example, Revanesse® Kiss for lip augmentation  typically lasts around 6 months before patients opt for a touch-up. With the lips being a high movement area due to talking, eating, etc. the dermal filler wears down a lot faster than say the chin or the under eyes with Teosyal® Redensity II

Can you make dermal fillers last longer?

While the longevity of dermal fillers is influenced by various factors beyond our control, such as metabolism and natural tissue turnover, certain strategies may help extend their lifespan. Staying well hydrated, maintaining a healthy lifestyle with a balanced diet, regular exercise, and sun protection can contribute to optimal skin health and collagen production, potentially prolonging the effects of fillers. Additionally, avoiding habits like smoking and excessive alcohol consumption can help preserve skin elasticity and overall vitality. Plus, sticking to the recommended treatment schedule for touch-up sessions or maintenance injections can help sustain volume and contouring over time. 

What happens when dermal fillers wear off?

When dermal fillers wear off, the injected product gradually breaks down and is naturally absorbed by the body over time. As the filler material dissipates, the treated area may gradually return to its pre-treatment state. Contrary to what many believe, wrinkles, lines and/or lost volume does not become “worse” than before if you stop receiving injections. Many will feel this way because fine lines or volume loss will appear more noticeable after seeing their face a certain way for a long period of time. 

Do dermal fillers ever wear off completely?

There is new evidence to suggest that some dermal fillers can remain in the skin for up to 15 years, suggesting that dissolving your filler manually may be required if the filler doesn’t break down naturally. According to a study in JPRAS Open, with dermal fillers being evident in patients years after their initial injections and expected product lifespan, dissolving filler with hyaluronidase may be required. While something to be aware of, this doesn’t mean that dermal fillers are unsafe. It simply means we need to be vigilant in approaching dermal filler with a holistic mindset. What does that look like? It may mean that we need to dissolve your lip filler before adding another syringe. While postponing achieving the results you desire can be frustrating, addressing filler migration and starting fresh after dissolving, will ensure better, safer results in the future. 

What happens after years of getting dermal fillers? 

While dermal fillers are generally considered safe (short-term side effects such as bruising, swelling, or redness are common and usually resolve quickly), the potential risks of long-term filler use are not yet fully understood. Some concerns include the risk of tissue damage, migration of filler material, or the formation of granulomas (small lumps) in response to long-term filler presence. Additionally, repeated injections in the same areas may lead to overcorrection or undesirable changes in facial proportions over time. This is why I stress the importance of regular follow-up appointments and careful consideration of facial anatomy and treatment plans. I do not hesitate in saying no to patients who I don’t believe need or should have additional filler—I am looking out for the you today and the you in twenty years from now.
