Why do vertical lip lines form?

Vertical lip lines develop for several reasons:

  • Dynamic muscle activity: The orbicularis oris muscle surrounds the mouth like a sphincter. Every sip, straw use or whistle contracts this muscle. Over time the muscle overpowers the thinning skin, leading to tense, pursed lips and vertical wrinkles. Cleveland Clinic notes that the underlying cause is the activity of the muscle around the mouth, which becomes more active with time.
  • Skin thinning and collagen loss: With age we lose collagen and elastin, the proteins that keep skin supple. The perioral skin becomes thinner and drier, making repetitive creases more permanent. Sun exposure and smoking accelerate this process.
  • Repetitive habits: Repeated pursing motions: sipping through straws, smoking, or frequently playing wind instruments

These factors explain why perioral lines often appear in the 40s, and why those who frequently purse their lips may notice them earlier. Static lines form when dynamic creases remain even at rest.

Why I choose Redensity II for vertical lip lines

Many dermal fillers can be used around the mouth, but I prefer Redensity II for these delicate vertical lip lines. Although this filler was originally designed for tear‑trough rejuvenation, its properties make it ideal for delicate perioral skin:

  • Low water attraction: Redensity II has a light, cohesive gel that attracts less water than many other hyaluronic‑acid fillers. In delicate areas this reduces the risk of puffiness or “duck lips,” a common concern for patients.
  • Hybrid formulation: The product combines cross‑linked and non‑cross‑linked hyaluronic acid with amino acids, vitamins and antioxidants. This dermal‑restructuring complex not only provides subtle volume but also supports skin quality.
  • Tissue integration: Redensity II integrates with surrounding tissue rather than sitting on top, so it rarely causes lumps or the bluish Tyndall effect. Dr Hassan Galadari emphasises that it is relatively non‑hygroscopic, meaning it doesn’t pull in extra water and therefore requires no over‑correction.
  • Small amounts, natural results: Because Redensity II is subtle, most patients need only 0.5 mL total for periorbital work. Around the mouth, I use equally conservative volumes to smooth vertical lip lines without adding bulk.

These qualities allow me to treat etched lip wrinkles while maintaining natural lip movement. I often combine filler with neuromodulators ( botox) to relax the orbicularis oris muscle and further soften lines, especially in patients who frequently purse their lips.

My Personal Transformation with Redensity II

My own transformation is so  important for me to share because so many of you have come to me for help with vertical upper lip lines in the past and I’ve turned you away (sorry, not sorry). Since then, Redensity II has been clinically proven to treat this tricky area and I’ve been loving the results that I’ve achieved personally and with a handful of patients so far.

  2. Discussion of habits and skin health: I ask about repetitive habits like sipping through straws or smoking, sun exposure and skincare. Avoiding repetitive motions and using broad‑spectrum sunscreen can help prevent new lines.
  3. Precise, conservative injections: Using a fine needle, I place tiny threads of Redensity II into superficial perioral lines. Because the filler integrates smoothly, I can treat individual etched lines without overfilling.
  4. Aftercare and hydration support: I advise patients to stay upright for a few hours (if neuromodulators are used), avoid excessive lip movement for the first day and hydrate the skin externally and internally. Ice can be applied briefly to reduce swelling; warmth may be used after three days if minor bruising occurs.

What to expect and when to follow up

Redensity II provides immediate softening of vertical lip lines, but final results appear as minor swelling resolves over three to four days. Because this is a high-movement area, results typically last six months. That said, results do vary and I highly encourage patients to return for a two-week follow-up and again at six months.
